APPLICATION CHECKLIST

This is a check list to begin your enrollment process. Links to the forms are located at the bottom of the page.

Section A: Pre-Admission

• Phone interview with Admissions Counselor
• Onsite visit and tour with Admissions Counselor
• Onsite interview with Principal (Documents to bring to interview: Most current report card and standardized test scores)
• Onsite Student interview

Section B: Admission Application

• Admission Application
• Academic Questionnaire – completed by teacher
• Copy of student’s most recent Standardized Test Scores ( Grades 1-12)
• Completed Request for Student Records form
• Copy of student’s Immunization Record
• Copy of student’s High School Transcript (grades 9 -12)
• Copy of student’s Report Cards – please include last year’s and current year’s report cards
• Copy of any legal documents pertaining to custody arrangements (if applicable)
• Character Recommendation (To be completed by a Pastor, Coach or Mentor)
• Teacher Questionnaire (Must be completed by the primary teacher, or a core class teacher, i.e., Math, English, Science, or Social Science)
• Statement of Parental Support
